Position Summary
The Community Health Outreach Specialist (Promotora de Salud) plays a vital role in promoting health and wellness initiatives within the community. Reporting to the Health and Wellness Manager, this position is responsible for recruiting program participants, facilitating health-related workshops, conducting community outreach, and coordinating food distributions. The Promotora de Salud fosters meaningful connections with community members, provides culturally competent support, and empowers individuals to improve their health and overall well-being.
Responsibilities
- Coordinate program events and workshops, recruit participants, and lead program activities.
- Facilitate workshops, discussions, and activities on various health topics in both English and Spanish.
- Establish positive and supportive relationships with program participants, following up as needed to ensure continued engagement.
- Inspire participants to take an active role in managing their health and well-being.
- Conduct outreach at community events, delivering culturally appropriate health information.
- Represent the organization at events to build awareness and promote health-related programs.
- Organize and coordinate monthly food distributions, ensuring smooth operations.
- Assign volunteer roles during food distributions and provide guidance to ensure efficiency.
- Collaborate with community partners to support events and programs promoting food security.
- Collect program data and maintain accurate records to measure impact and outcomes.
- Provide reports to the Health and Wellness Manager as required.
- Attend required professional development and training opportunities to enhance skills and knowledge.
- Adapt to a flexible schedule, including after-hours and weekend commitments as needed.
- Perform other duties as assigned and work occasional evenings and weekends as needed.
Qualifications
- Associate’s degree in human services, public health, sociology, or equivalent employment experience.
- Experience managing a caseload or working in community outreach is preferred.
- Bilingual in Spanish and English (oral and written) is required.
- Excellent communication and customer service skills, with the ability to engage effectively with individuals from diverse backgrounds.
- Strong organizational and problem-solving skills, with attention to detail.
- Proficiency in Microsoft Office Suite and data entry systems; experience with data collection and reporting is preferred.
- A positive attitude and a genuine interest in assisting community members and program participants.
- Ability to work both independently and collaboratively as part of a team.
- Flexibility to travel and work a variable schedule, including occasional evenings and weekends.
Work Schedule
This is a full-time, onsite position with a Monday to Friday core schedule. The Community Health Outreach Specialist (Promotora de Salud) is expected to maintain a flexible work schedule in order to meet community needs. Evening and occasional weekend events may also be required to support the HCWM team and perform other duties as assigned.
